way of management after the surgical operation |
Many people believe that patient must not move
and just lie down after disk surgery. This is
not recommendable to lie down for more than three
days because it has considerable side effects.
Lying down for a long time can weaken the supporting
back muscles. Although the patient may get over
the disc-related disease, the lumbago can last.
Also, it can cause thrombosis because of poor
blood circulation and cause malnutrition with
the need for disk rehabilitation.
In addition, there can be complication such as
osteoporosis and urinary calculus caused by hyper-calcinuria
as electrolytes, such as bone calcium, flows off
into the blood. Therefore the patient should gradually
increase the amount of the activity after operation
and should start waist exercise in earnest about
two weeks after operation.

Correct
posture to prevent lumbago

- Sitting posture
Bad sitting posture is one of the causes of back
fatigue. A chair with a fixed high back which
can support the waist is good.
The patient seat deeply in the chair. Sitting
at the edge of the chair is not recommended because
weight is not distributed correctly when the waist
is not in proper posture.
- Standing posture
A good standing position is for one foot to come
before the other with the knees slightly bent.
Alternatively, place one foot on something slightly
raised while standing in place or when doing kitchen
work. You can also use a foot mat to avoid twisting
the waist.
- Sleeping posture
There are many times when we wake up with stiff
shoulders and necks. If you suffer from these
symptoms, you should check your sleeping posture
and your pillow. Both a too low or too high pillow
can burden the neck and shoulders.
Either a mattress or an air-cushion under-quilt
is recommended, and sleeping on the side is better
than on the back. Sleeping on the stomach increases
the curvature of the spine, so it is harmful to
the spinal posture as well as the lungs, which
are pressed.
- Driving posture
Recently professional drivers suffer from spinal
disease. That can be from the fatigue from extended
sitting in one position, but most are the result
of bad posture. And extended driving without break
can exert a vicious influence on the spine. Therefore
driving more than two hours is not recommended.
The good driving habit for the spine is to take
a 10 minute break per hour if possible.

There are many types of treatment for lumbago
patients; medical, thermic and traction treatment.
Among the most recommended treatment is rehabilitation
exercise treatment which increases the flexibility
to the muscles around the waist and which enhances
the solidity of the muscles. Muscle consolidating
training increases the solidity, fortitude and
size of muscles and the density of bone and of
cohesion tissue. Regular exercise intensifies
the ability of nerve control, and resistance training
protects our body from excessive contracting and
develops the sensory organs.
The best exercises increases for pliability is
stretching, and William abdominal muscle strengthening
exercise and McKennzy expansion exercise are also
effective. However these exercises have not been
evaluated objectively and do not have objective
standards.
Many kinds of computerized tools have been developed
to supplement those problems.

two months muscle size increases, and after about
12 weeks training muscles are at their strongest.

Effect of sports on the

Severe exercise make people who have back problems
worse. Exercise training can reduce the pain.
Patients over 35 years old or those with chronic
lumbago should be prescribed with the proper warm
us and cool down exercises before and after physical
activity. The following are helpful spine exercises
for normal life.
- Fast walking :
This exercise is helpful for everybody. It strengthens
the cardio-pulmonary functions and promotes blood
circulation and bowel activity.
This is also recommended for the balance of backbone
and endurance. A 10 minute 1 kilometer walk on
dirt or grass is recommended. Fast walking is
more recommended than swimming.
- Cycling :
Pedaling puts the pelvis in the correct position
and develops the muscles around the buttocks and
legs.
But if the saddle is too high, it can have harmful
effect on the waist; the saddle should be adjusted
to the persons height.
- Swimming :
The resistance and buoyancy of water can lessen
the pressure on the waist and is good in preventing
joint injury. This is recommended for the middle
aged.
The backstroke and free-style are best, but the
butterfly and breast strokes should be avoided
if waist muscles are not strong enough. 20-30
minutes 2 or 3 times a week per is effective.
But swimming in water below 21C causes muscles
to contract and should be avoided.
There can be no more important activity than
sex for human emotional life. Even Lumbago and
hip rheumatism patients cannot avoid sex.
Is bed rest necessary for urgent and chronic
lumbago?
When people suffer from sudden lumbago, three
days of bed rest is necessary. After about 3 days
sex is possible and can be helpful for the lumbago.
Especially in repeated cases of chronic lumbago,
an active sex life is more necessary than bed
rest or the avoidance of sex.
What role does sex serve emotionally and psychologically?
Physical intimacy and comfort can help patients
emotionally and psychologically. Although sex
may be impossible because of severe pain, mutual
touching and petting with hands and lips are recommended.
What effect does touching have on pain?
Actually mutual touching with an open mind can
have a positive effect for the pain and the nervous
system.
Is back stretching exercise the best way to prevent
lumbago?
The best way to prevent lumbago and recover from
lumbago is active exercise and not bed rest. The
most common stretching exercises to strengthen
the waist muscles are pelvis lifts and extension
exercises.
These two exercises are possible during sex. Waist
extension exercises and pelvis lifts can all be
exercised during sex, and can strengthen the back.
When sex activity is possible for people who
have had back bone surgery?
Anytime after the surgery scar has healed. On
the basis of walking, if the patient can walk
1.5km. or 2km. without pain, sexual activity is
definitely possible.
The effects of pregnancy on the back is a popular
area of research. According to the research reports,
55% of Korean women suffer from lumbago during
pregnancy, and lumbago starts from the third trimester
of the pregnancy.
The cause of the lumbago in the last period of
pregnancy is that the waist is burdened by the
weight of fetus. As the womb expands, the other
abdominal organs are forced forward and thus deforming
of the lower back.
Women suffering from lumbago during pregnancy
should rest and wear a back support corset if
needed. Women following these guidelines should
not have much lower back problems after delivery.
There is danger to the waist after delivery.
Therefore post-partum care is important. Women
with lumbago after delivery should use the correct
posture during physical activity and backbone
strengthening exercise if necessary for back training.
Carrying the baby in the arms is highly prohibited.
Women should carry the baby on the back or use
a rucksack or baby carrier. When lifting the baby,
she should not bend her waist but bend her knees
bring the child to her and straighten her knees.
This method should not cause stress to the lower
back.
Wearing unfit shoes deteriorates lumbago, and
the worst are high-heels and slippers. High-heels
cause excessive stress of the waist joint, and
shoes without heels, such as slippers, tighten
the foot causing lumbago. Good shoes are stable,
elastic and absorb ground impact.
The base of the shoe should be stable, flexible,
tender, and should have a solid grip on the ground.
The side of the shoe should curve along the shape
of the foot and support the arch. Slippers and
sandals are not beneficial as they do not protect
the sides and heel of the foot.
The heel of the shoe should be single, wide and
below 5cm. in height. Sports shoes are good. A
shoe should have with room in the toes, fit tightly
in the heel, support the sides, and cushion the
foot. Placing a scrap of cloth to function as
a cushion inside the shoe, is good for lumbago
and arthralgia.

Action of Food, drinking
& smoking on the spine

It is a medically proven fact that excessive
alcohol consumption causes the body to lose calcium.
If calcium leaks out from the spine, it leads
to osteoporosis and weakens the spine. Moreover,
the ligaments and muscles supporting the lower
back joints and disk can be damaged. Alcohol consumption
is very bad for the disk patient.
Smoking can also have a negative effect in the
deterioration of the spinal disk. Smoking can
also give rise to coughing caused by chronic bronchitis.
Coughing increases pressure inside the abdomen
and the disk, so can be a second-hand factor in
damaging the disk.
Heavy smoking reduces the mineral content of
the spine and lowers the density of bones so it
is likely to effect the disk. Nicotine makes vascular
contraction and reduces the circulation of the
blood to the backbone. Therefore, it hinders the
supply of nutrients and can lead to degeneration
of the disk.
Stimulant beverages such as coffee and coke,
causes the leak of calcium from the backbone.
Obesity should be avoided. Obesity causes overextension
of the lower back and disturbs the backbone. Therefore,
balanced meals with low in fat and with abundant
vitamin and mineral are recommended.

Diseases which cause paralysis
of the body |
- Cervical Disc Herniation
Problems with the cervical disc herniation are
more dangerous than with the spinal discs because
problems with cervical disc can cause paralysis
of the arms, legs and the body.
The process of paralysis from a cervical disc
herniation can be so slow and different for each
patient that doctors may not diagnose it as a
cervical disc herniation.
Although the cervical disc herniation can be
cured by surgery, it can be seen as an incurable
myelitis or a change of the spinal cord itself,
and can be misdiagnosed as a spinal tumor, so
there may be cases where surgery is not performed.
Therefore a precise diagnosis is absolutely necessary.
A patient with paralysis may be cured with surgery.
- Cervical posterior ligament osteosclerosis
Since publication in Japan in 1960 of research
on posterior ligament osteosclerosis causing paralysis
and neuralgia because of pressure exerted on the
spinal nerves, many cases have been reported in
Korea. Such cases occur in Japan because they
bow very often, and it occurs in other Asian countries
including Korea, as well.
- Tuberculosis, tumor and abscess of the spine
Spinal tuberculosis and spinal tumors causing
paralysis of the lower half of the body are not
rare in Korea. And in some cases are misdiagnosed
as spinal herniation and so do not receive the
proper treatment. This can often cause nervous
trouble. Spinal tuberculosis is a problem in Korea
as there is a tendency to overuse adrenocortical
hormones (steroid).
Early spinal tuberculosis does not influence
the nerves, only the lower back. If it is not
treated in time or there is abuse of adrenocortical
hormones, the lower half of the body can be paralyzed.
Once paralysis occurs, surgery should be performed
as soon as possible.
Spinal tumors need to be operated on early, but
is often misdiagnosed as disc herniation and can
cause paralysis. If there is continued lumbago
with no discernable improvement after two weeks
treatment, reexamination is necessary. Pus should

infection, before there is serious nerve paralysis.
